Could you make another for his Grievous wound? You roll for each injury individually in HM. This one will be vs 62 as the treatment for a G4 cut is +10 EML.
Here goes for the S3 to the TX. You get +20 to your EML so your Physician ML +20 to treat a S3 Cut with Surgery (sew him up basically). 52+20=72 target, your 75 just missed and is a Critical Failure.
So his S3 TX has a Heal Rate of H3 and it is infected (due to the CF).
You also rolled a CF so you get to make a Skill Improvement roll for Physician of %d+Physician SB of 13. If it is greater than 52 your Physician ML goes up to 53.

Idril grinds her teeth in frustration as she tries to aid her patient. She snaps at the guard and the new comers, "How am I expected to concentrate with these interruptions?" Glaring at Arngeir, she says, "You know your way around the mortal flesh? Fine, see what you can do. Meanwhile, I'm going to go out in the woods and invite a dozens squirrels to come in here and chatter as you work, see if it steadies your hand."

Arngeir looks over the injured man and notes he is badly injured with a cut to the chest (S3 TX H3, infected), a cut to the thigh (G4 TH), and at least some bloodloss, although not as bad as you would expect based on the cut on his thigh. He is going to need to try and deal with the infection first (Heal option under Psionic Healing is his best option, Idril is the better Physician I think to tend him but you don't know that yet).
Algrim walks around checking things out and the merchant approaches him, "Hail good Sir, I am Sax of Dor. Some of the Foulspawn got away last night. We are hoping we will be able to head back for Kobing soon in case they are back tonight."
Idril got a Marginal Success on the G4 which is a H4 healing rate and a permanent loss of 1d3 Agility once he heals as this was a really bad injury (you did well here actually, it could have been a lot worse). Your Physician is now 53 ML.

Algrim nods his head "Foul orcs, er I guess you call em Foulspawn. I yer plan is a wise one. If any orcs show up I'll give em somethin ta run from." the dwarf makes a threatening gesture towards the woods pretending he sees an orc.

The Ivinian crouches in front of the injured man and lifts a symbol from under his shirt. He starts to recite and repeat a litany in what must be his home tongue

He opens his eyes and places his hands gently on the infected wound, once again he closes his eyes and his brows knit in concentration:

You nailed it. You get the +50% to your EML so you have a MS on the Psi Healing and a Skill Improvement roll for Ritual (%d+SB). You have acquired 2 Fatigue Levels from using Psionic Healing. 40 minutes of rest (not doing anything stressful or 20 minutes of sleep will remove them. They do modify additional Psionic rolls and Invocations so I recommend resting before trying again. His infection has improved to a H4, reminder he was at H3, H0 is dead and H6 cures the infection. He gets to roll to heal on his own once per day.
As Arngeir prays and holds his hands over the infection it visibly improves. The color seems to fade some and it seems to contract some. The man sputters and gasps, but he quickly stops and his breathing seems better.

Idril looks like she is about to murder Arngeir when he tells her to shush, but she pauses to watch his ritual, and her expression softens slightly when she see's the patient's infection visibly recede.
"He should be able to rest better, now. You did well by him, priest."
Glancing over at the dwarf, boasting about his people's prowess in the middle of the ritual, and says to Arngeir, "Can you vouch for that one?"
Standing up from beside the patientand dusting off her skirts, she addresses Algrim. "You, khuzdal. Not much market for locksmiths, here, so I hope you don't object to finding other ways to be helpful. There's stew on the fire. You can grab a ladle and pass it out, I expect."

She turns her attention to a basket of shriveled parsnips, muttering curses at the brute of a farmer who did such a poor job harvesting them.

Idril gets Algrim to help her serve a late breakfast as Arngeir rests and recovers from his mental exhaustion. After handing out the food and having some to eat himself Algrim notices one of the guards waving him over.
About an hour has gone by, Arngeir is rested, sitting with Idril by the patient.
